Your initial appointment will consist of a consultation discussing your symptoms, concerns and existing conditions, as well as a clinical examination. Occasionally, procedures (e.g., a biopsy) may be performed the same day as the consultation or perhaps X-rays will be taken. In other instances, blood tests and/or advanced imaging tests (e.g., MRI, CT scan, etc.) may be ordered to help establish the diagnosis. Depending on the diagnosis, treatment often requires additional follow-up appointments.

Paperwork to Complete

Prior to your visit, please print out and complete the required “new patient” forms. Copies of all new patient forms are found on this website under the Forms tab. (You can also reach our new patient forms, by clicking here.) If you cannot access the forms online, please call us and we will fax or mail a set to you. You can fax or mail these forms to the office where you will see Dr. Laudenbach before your visit or bring the completed paper work with you. Feel free to call the office with any questions that you might have.

Other Needed Items

Depending on your circumstances, you will be asked to provide certain items that are relevant to your case.
These might include:

All previous biopsy reports, blood test results
All previous dental and/or medical imaging films, scans, CD-ROM disks, etc. (e.g., MRI, CT scan, bone scan, Panorex,  individual and/or complete series of dental films) with associated written radiology reports
Medication list, including dosages and any over-the-counter medications you are taking
Appropriate consultant letter(s)/clinical notes (e.g., from your otolaryngologist, periodontist, oral surgeon, dermatologist, etc.)

Please gather these items and bring them with you to your first visit or fax them in advance. If you would like us to call your current dentist or doctor for copies of your X-rays or other information, please contact the office where you will see
Dr. Laudenbach at your earliest convenience. We will likely need to obtain written consent from you to facilitate the transfer of your medical/dental documents.
